The Multiple Chronic Conditions Resource Center (MCCRC), provides health care professionals and patients with the current trends in health policy and clinical practice to meet the complex care needs of the largest patient population – those living with two or more symptomatic chronic conditions.

In 2010 the U.S. Department of Health & Human Services initiated the Multiple Chronic Conditions Strategic Framework. This initiative unfolded at the same time as the unveiling of the Affordable Care Act. The MCCRC was created to assist clinicians and the public with easily accessible FREE information and resources to promote best practices in patient-centered care.

Managing chronic conditions and associated symptoms prevents or reduces decline, disease progression, and debility allowing patients and families to engage in meaningful and important activities.
